摘要(Abstract)：

目的总结走进长者服务中心药学服务工作的经验。方法以参加社区综合服务中心的社区长者和其家人或陪护为服务对象,以药学服务形式介入社区长者服务中心的日常活动,通过讲座、用药咨询、派发用药小册子等形式,为长者提供各种日常用药方面的指导。结果经过开展系列药学服务,服务中心的长者们掌握了更多药物的相关知识,提高其用药的合理性与有效性。结论药学服务的实施激发了社区老人对药安全性的重视,间接使其日常生活质量得到进一步的改善,进而为构建和谐的医患关系打下良好基础,所以应重视老年长者为主的药学服务,建立起长者综合服务中心老人规范的用药指导。
Objective To summarize the experience of the pharmaceutical care in the elderly service center. Methods The community elders and their families or escort who participate in the comprehensive community service were the service object,in the form of pharmaceutical care getting involved in the daily activities of the community elderly service center,providing various daily medication guide for the elderly through lectures,drug medication consultation,distributing brochures and other forms. Results The elderly who participated in the community service center can master more information about drugs,and the rational application of drugs and their effectiveness were improved. Conclusion The implementation of the pharmaceutical care inspired the emphasis on drug safety of the community elderly and indirectly improved their life quality,and lay good foundation for the harmonious relationship of doctor-patient. The pharmaceutical care of the elderly should be pay more attention to,and the standard guidelines of drug application of the elderly in comprehensive service center should be established.
