摘要(Abstract)：

目的通过讨论本院药物治疗管理(MTM)门诊服务模式与流程,总结分析MTM分级实施情况及存在的问题。方法将MTM服务分为三级,分别对各级别的主要内容、工作方式,人员要求进行分析,并对各级别工作内容、流程、随访和复诊、工作评估等方面各种情况进行讨论。结果药学门诊MTM分级实施规范管理包括制度和流程、文件记录管理及质量控制等环节,更好地发挥药师的专业服务功能。结论 MTM分级实施能优化MTM服务的工作流程,节约药师出诊时间,提高MTM服务的工作效率及服务质量。
OBJECTIVE To discuss the service pattern and process of Medication Therapy Management( MTM),and to summary the classification implementation and hierarchical management probleml for MTM. METHODS MTM services were divided into three grades. The classification implementation of pharmacist-managed clinic work content,process,work ways,staffs in MTM services were analyzed. And the pharmacist-managed clinic work content follow-up and assessment,job evaluation were discussed in these three grades,respectively. RESULTS MTM classification implementation of pharmacist-managed clinic management,facilities,including the service object and scopes of visits pharmacist access,system and processes,files,records management and quality control,made the medicine outpatient norm and better play to the pharmacist professional services. CONCLUSION Different forms of MTM classification implementation of pharmacist-managed clinic optimize workflow efficiency,save the pharmacist call time increased the quality of services.
