社区患者高毒型幽门螺杆菌的耐药性研究

摘要(Abstract):

目的了解所研究的社区人群中,高毒型幽门螺杆菌(Hp)的体外耐药性,为临床治疗提供参考依据。方法胃镜检查时用快速尿素酶初筛出Hp阳性患者,通过Hp抗体分型确定所携带Hp菌株是否为高毒型,对高毒Hp患者的胃黏膜标本进行体外培养,用E-test法检测Hp对常用抗菌药物的体外敏感性。结果共103例患者进行了胃镜检查,65例Hp阳性,其中51例携带了高毒型Hp菌株,占78.46%,27例体外培养成功,药敏结果显示,Hp对阿莫西林、呋喃唑酮、四环素的耐药水平较低,分别为7.41%、0%、11.11%,对克拉霉素、甲硝唑、左氧氟沙星耐药率较高,分别为51.85%、81.48%、70.37%。结论高毒型Hp菌株对常用抗菌药物的耐药率差异较大,临床医生可根据耐药结果为患者提供个体化药物治疗方案,提高高毒型Hp的根除率。
OBJECTIVE To understand the in vitro drug resistance of high-toxicity helicobacter pylori( Hp) in the community patients,and provide reference for clinical treatment. METHODS The Hp-positive patients were screened out by rapid urease during gastroscopy,and the Hp-bearing samples were determined to be highly toxic by Hp antibody typing. The gastric mucosa specimens of patients with high-toxicity Hp were cultured in vitro,and the in vitro sensitivity of Hp to commonly used antibiotics was detected by E-test. RESULTS A total of 103 patients underwent gastroscopy,and 65 patients were positive for Hp. Among them,51 patients carried highly toxic Hp strains,which accounted for 78.46%. 27 samples in vitro culture were successful. The drug susceptibility results showed that Hp had low levels of resistance to amoxicillin,furazolidone and tetracycline,the drug resistance rate were 7. 41%,0%,11.11% respectively. However,the resistance rates of Hp to clarithromycin,metronidazole and levofloxacin were higher,the drug resistance rate were 51.85%,81.48%,70.37%,respectively. CONCLUSION High-toxic Hp strains have different resistance rates to commonly used antibiotics. Clinicians can provide individualized drug treatment programs based on drug resistance results,and improve the eradication rate of high-toxic Hp.
